The men's 400m medley relay final took place today at the Tokyo Aquatics Centre in Japan.

Team Korea—consisting of Lee ZU HO (Seogwipo City Hall, backstroke), Choi Dong-yeol (Gangwon State Government, breaststroke), Yang Jae-hoon (Gangwon State Government, butterfly), and Kim Young-bum (Gangwon State Government, freestyle)—combined for a time of 3:31.97, finishing third behind China (3:27.89) and Japan (3:28.70).

Having won the silver medal in this event at the 2022 Hangzhou Asian Games, South Korea reached the podium for the second consecutive tournament.
